• Student Physicals

    Education law and regulations of the Commissioner of Education require physical examinations of children. Beginning in September, the school physician will conduct physicals on the students attending Brookfield Central School. (Parents have the option of going to their own doctor for their child's physical at their own expense.) Health forms will be available in the health office if a parent decides to use the child's own doctor. If the health forms are not returned and/or a date scheduled with your own physician, the school physician will perform a physical.

     

    Students in Grades PreK, K, 2, 4, 7 and 10 are required to have a physical during the school year. The health exam may be performed by your own private physician or the school physician.

  • Authorization for Administration of Medication

    During the course of the school year, it may become necessary for your child to take medication(s) during schol hours due to colds, sore throats, ear infections, allergies, etc. Even over the counter medications such as Tylenol requires a prescription from the student's Primary Health Care Provider along with parent permission. Please contact the school to obtain this form. This form can also be found in the school calendar.

  • Dental Health Certificate

    There is a sample dental certificate available for you to take to your child's dentist and once it's completed, it should be returned to the School Nurse as it will be filed in your child's Cumulative Health Record. Please be advised that the Dental Certificate is not a mandated requirement to attend school.

  • Student Emergency Information

    Please complete the attached Student Emergency Form and return to the school nurse. Be sure to list two neighbors or relatives who live close by you and are available during the day. It is important that you let them know that you are using them as an emergency contact. This form also requires you to choose a physical at school or by your private physician (indicating date of exam).
